Advance care planning helps identify, communicate and document the health care decisions you would make if you could not speak for yourself in a health crisis. Having these discussions in advance helps adults of all ages to prepare for unexpected situations so medical providers and loved ones can honor their health care wishes. These decisions are recorded in an document called an advance directive. Here in Santa Barbara County, we recommend using the MyCare advance directive.
